If you engage in any Heart Comonos Events, you agree that you do so at your own risk, assume all risk of injury to yourself and agree to release Heart Comonos from all claims or causes of action, known or unknown arising out of this activity. I acknowledge that photographs and/or video will be taken at Heart Comonos Events, and I agree to them being used on the Heart Comonos website and social media sites. I acknowledge that I have carefully read this “waiver and release” and fully understand that it is a release of liability. I expressly agree to release and discharge Heart Comonos from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action against Heart Comonos for personal injury. To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence. If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from. By signing this release, I acknowledge that I understand its content and that this release cannot be modified orally.

1. Every precaution will be taken for the safety and good health of the participant but in the event of an accident or sickness by whatever cause, I will not hold Heart Comonos staff, volunteers, affiliates, or Event Coordinators liable for any damages, loss or injuries caused to my minor participant.


2. I agree to allow the named participant to take part in all Heart Comonos Event activities unless otherwise stated on this form. 


3. ***For participants volunteering without parent supervision during the duration of any Heart Comonos Event, I consent to allow the named participant to take part in all the Event activities without my supervision at each Event. I release Heart Comonos from all claims or causes of action, known or unknown arising out of this activity. I acknowledge the participant can be asked to volunteer with the Heart Comonos Team, before, during and after the Event. I am responsible for arranging drop-off and pick-up accommodation for my minor participant. 


4. In the case of an emergency, I understand that every effort will be made to contact myself or the designated Emergency Contact Person.


5. I agree that the named participant and I will assume liability for any damage to property proven to be caused in part or whole by the participant named.


6. I understand that the Heart Comonos staff reserves the right to dismiss anyone who in their opinion is a hazard to the safety and rights of others, or who appears to have rejected the reasonable controls of this Event.
